If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient. 


No Shows

1st no show - courtesy
2nd no show - $35.00 fee to be paid before your next appointment
3rd no show - Additional $35.00 fee or possible discharge from the practice

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit.

What to Bring to your Appointment

  • Insurance ID Card and Drivers License
  • Referral Forms (if applicable) - A referral must be in the system in order to be seen.
  • Previous Health Records including any x-rays, MRIs and/or test results that may pertain to your treatment.
  • List of current medications
  • A pair of shorts if your problems pertain to the leg, knee, ankle, or foot
  • If you are being seen as a result of a Motor Vehicle Accident or a Workmans Comp Claim, you must bring the following information:
    • Claim number
    • Address and phone number of the insurance company
    • Adjuster or nurse manager name and phone number
    • Must present with back-up health information or you will not be seen

Note: All patients under 18 must be accompanied by a parent or guardian. Also all patients coming to us from nursing facilities need to be accompanied by a family member or a health care aide familiar with the patient’s issues, the language and be able to complete any required paperwork for the patient


Billing & Payment

Co-payments, co-insurances and deductibles are due at the time of visit.
We also accept the following credit cards:

  • Discover

Health Insurance Plans

We participate in Medicare, most other insurance plans, and many managed care and HMO's. Always bring your insurance card(s) for each appointment. Always keep us informed of any changes to your phone number, address or insurance. Covered procedures will vary with each insurance company -- If you are unsure if a visit or procedure is covered, you should check with your insurance company..

X-ray facilities are on site and are available to patients pursuant to their insurance company. However, some insurance plans (including many HMOs) require its members to have their x-rays done at a predetermined location based upon contracts they have entered into. Patients with these plans can not have x-rays done in our office. It is the patient’s responsibility to bring these x-rays with them to their appointment.

If you have been referred to us from your primary care physician or belong to a managed care or HMO program, you are responsible for making sure you have proper authorization and referral forms for all your visits. WITHOUT AUTHORIZATION, WE WILL BE UNABLE TO SEE YOU. This is the policy of your insurance company.

It is best to confirm with the office at the time you make the appointment if your health insurance is accepted


Updating Records

It is very important that your doctor have your up-to-date address and phone number. In case of an emergency we must be able to contact you. Please make sure the receptionist is aware of any changes. Also, please make sure we have both work and home phone numbers and another emergency contact number if possible.


Requests for X-rays and Refills on Prescriptions

Copies of X- rays are available for patient consultations; however 48 hour notice is required before pick-up.

Any requests for prescription refills should be directed to the appropriate physician assistant. Please make sure that the following information is provided - name of patient, name and phone number of the pharmacy, and the prescription to be refilled. Please allow 48 hours notice.
